一例待行二尖瓣置换术患者的循证治疗

病例背景：

患者女性，
45
岁。





主诉：呼吸困难
1
月，加重
1
周，痰中带血
2
天。

现病史：
1
月前，
患者做家务后出现呼吸困难，
休息后可缓解，无喘息喘鸣、
咳嗽咯血、
胸闷胸痛，当时未予处理，此后上述症状持续并加重，

周前咳嗽较频繁，为呛咳，单声，
咳白色粘液痰，快步行走及上楼梯时出现呼吸困难，无夜间阵 发性呼吸困难，

天前无诱因
下咳出粉红色痰，无发热、无恶心、呕吐，无腹痛腹胀， 无肛周疼痛。于当地医院就诊，查
胸片示“肺水肿”
，患者为求进一步诊治，于今日到我院急诊 就诊，查心脏彩超示“二尖瓣
狭窄并关闭不全”
，收入我科。患者自起病以来，大小便正常，饮 食可，精神可，睡眠可，
体重无明显减轻。

体格检查：体温：
37.2℃；脉搏：
95
次

分；呼吸：

次
/分；血压：
128
/72mmHg
。一
般情况尚好，自动体位。
皮肤巩膜无明显黄染，肺部听诊双下肺广泛湿罗音，心脏听诊心尖
处闻及收缩期吹风样杂音舒张期隆隆样 杂音，
向左腋下左肩胛处传导。
腹部平坦，
腹无压痛、
反跳痛（
-< br>）
，肝脾肋下未触及，未及腹部肿块。双侧下肢无水肿，神经系统体征（—）
。肛
门及外生殖器未查。

入院后检查：
心脏多普勒超声示：

B:
左房增大，余房室比例正常，二 尖瓣瓣环增厚回声增强，
，二尖瓣三尖瓣关闭裂隙。
肺动脉内径增宽。





CDFI:
收缩期二、三尖瓣瓣上呈以蓝色为主五彩镶嵌的 返流束。根据连续多普勒三尖瓣
返流压差估测肺动脉压约
46mmHg
。


一、提出临床问题

患者先内科治疗缓解肺水肿，
后需经外科治疗 根治，
现认为该患者需行二尖瓣置换术，
在不
考虑经济条件情况下，可以选择机械瓣膜 或生物瓣膜。两者差异是：机械瓣膜使用时间长，
而且不易劳损，短期再次手术机会小，
20< br>年后可能需要更换，但需终身服用华法林；生物
瓣膜术后不需抗凝，
视个体差异可能终身 无需再次手术，
或者因排斥会在短期内出现钙化需
要再次手术。
